New Employee Checklist - a printable checklist of forms to file and other items to square away when you're first hired

New Employee Account Request - Use this form to request that a new employee's computer accounts and services be set up. The employee's supervisor or a member of HR must fill this out.

Required Payroll Forms

The following must be completed by the new employee in Workday:

  1. Form I-9 - Employment Eligibility Verification
  2. W4 Form - Federal Withholding Elections
  3. Affirmative Action Form
  4. Direct Deposit Form

Important UW Benefits Deadlines

To sign up for benefits, turn in your forms by the following deadlines:

Benefit Forms Due Within
Medical & dental insurance 31 days
Long term disability insurance 31 days
Term life insurance 60 days
Retirement plan 90 days

We suggest making a copy of each form for your personal records.
