New Employee Account Request

To request computer accounts and services for new employees (including temporary workers and student workers), the new employee's supervisor or a member of the HR office must complete the following form and submit it to the Computing Services department.

We generally require 2 weeks' notice so that we can ensure the new employee's computing environment is ready for when he or she starts.

A member of Computing Services staff will contact the requestor with instructions, username, etc. shortly after we receive the information.

Note that a UW NetID is required for an Exchange account. It takes up to 2 business days for UW-IT to create the exchange account. We cannot request the exchange account to be created until we have the new employee's NETID.

If you do not have this or know what it is, the new employee must go to and create their NetID. Anyone with an Employee ID is eligible for a NetID.

Accounts and Services Requested

Windows Logon
Will be the same as UW NetID
File shares or drives user should have access to (e.g. "TC, Develop, Common, CWD, CELT"):
Exchange (E-Mail/Calendar) Account
Optional. The Exchange account service is maintained by UW Technology and requires a NetID/Supplemental Account. It costs $8/month, so please ensure that the employee really needs a calendar account. (Standard email accounts are free does not include calendar.) Also note: students receive departmental supplemental accounts rather than Exchange accounts.
Budget for Exchange
Please specify a budget number to charge for Exchange services.

Email Lists

Indicate which of the following College administrative email lists the new employee should be a member of:
all College staff
all College faculty

Staff in a Dean's Office unit or members of CWD, GenOM, MESA, or Computing Services

Indicate the names of any other mailing lists in the namespace the new employee should be a member of (one per line):

"Generic" Accounts

Sometimes it may seem to make sense for positions of high turnover (student assistants) or in support roles (front desk staff) to use "generic" accounts, such as "frontdesk" or "student1." The use of generic accounts, however, presents a problem of accountability and weakens the overall security of the network and we strongly discourage their use.

Generic Windows logon accounts will no longer be created. Windows logon accounts may not be shared or transferred.

Generic email addresses and generic Oracle Calendar accounts can only be created if you request a supplemental NetID from C&C. After you obtain a supplemental NetID, you may contact Computing Services ( and we can help setting up the email account and/or Oracle Calendar account using that NetID.
