Nominations for the 2020 Diamond Awards are being accepted now through Friday, October 11, 2019. You may submit multiple nominations.

To complete your nomination:

1. Download the Nomination Form.
2. Fill out all applicable fields and save the completed form using Adobe Reader.
3. Send an email to Deirdre Sumida at deirdres@uw.edu with the following documents attached:
   • The completed nomination form, which includes your personal narrative of the nominee's achievements
   • The nominee's vitae, resume or biography
   • 3-5 letters of support for the nomination addressing award criteria and nominee's merit.
