Conviction/Criminal History Background Checks

Overview

The University of Washington Conviction/Criminal History Information Form and criminal background review are used during the advertising, solicitation and offer stage of a recruitment for a paid or volunteer position. Use of this form and the criminal background check is not warranted for every position, but must be used in situations when a new employee will have unsupervised access to vulnerable populations. More information regarding this law can be reviewed here: RCW 43.43.830 through 43.43.842. Hiring managers are responsible for deciding when a recruitment will require completion of the form and background check, and should contact the Human Resources office at 616-2604 if they have questions.

Working with your Employment Specialist

Your Employment Specialist will assist you with the coordination of your recruitment for a paid position, and can assist you with background checks for volunteer positions. When recruiting volunteers or paid employees, you should indicate to your Employment Specialist when applicants will be required to complete the Conviction/Criminal History Information form. This fact should also be mentioned in the text of any recruitment materials for the position, including posting on the UW's web pages, fliers, and any web or print ads. When recruiting volunteers, contact your Employment Specialist before your recruitment for help in determining the best process for conducting the background checks.

The individual who receives applications from candidates will need to ensure that the Conviction/Criminal History Information Form has been completed by every candidate, since candidates who do not complete the form cannot be considered for the position.

As you near completion of your recruitment, contact your Employment Specialist regarding completion of the background check(s) you require. The Employment Specialist will require the Conviction/Criminal History Information forms to complete the investigation.

The Personnel File

Conviction/Criminal History Information forms are forwarded to Upper Campus Operations, and should not be maintained in department/program personnel files
