Community of Innovators Awards
Submitting a Nomination
We are no longer accepting nominations for the 2009 awards. Please check back in January 2010 for information regarding nominations for next year.
Students, staff and faculty members with valid UW NetIDs may submit one or more nominations using an online form that is available only during the nomination window.
Preparing Your Nomination
We recommend you gather the following information before starting a nomination form:
- Information about the nominator (you).
- Information about the nominee: their name and department and your relationship to the nominee.
- Your nomination. You can cut-and-paste from a Word document or type directly into the space provided on the form.
- Two or three letters of support in addition to your original nomination.
Letters of support need to come from someone other than the original nominator and preferrably from more than one constituency. Internal and external colleagues, department chairs, institutional representatives, students (undergraduate, graduate and post-docs), staff and faculty members are examples.
Important Considerations
- Incomplete nominations will not be considered.
- Nomination materials may be shared with nominees and/or included in personnel files, and may also be edited from their original appearance and/or length.
- If you are unable to access or use the online form due to a disability, or if you do not have access to a computer, please contact Brittany Sample, (206) 616–9808.
