Tips for Filling out Nomination Forms
The Basics
Note: we are now requiring two (2) letters of support for each nomination.
- Nominations are submitted via web-based forms, available above when the nomination window is open.
- Student, staff, or faculty members with a valid UW NetID may submit a nomination.
- Nominations will be accepted for a limited time, called the "nomination window."
- There is no limit to how many nominations you may submit, but please note that in any given award category, if you have a nomination saved you may not start another one until the unfinished one is submitted.
- Hard copy nominations will only be considered from individuals who require specific accommodation due to a disability or other condition which prevents them from easily accessing and using web-based technologies. If you require such an accommodation, or if you do not have access to a computer, please call the College of Engineering Dean's office at 206-543-0340 or stop by Loew Hall, Room 371.
What the Nomination Will Look Like
The online nomination form is comprised of four (4) required sections:
- Information about the Nominator (you)
- Your name
- Your role (faculty, staff, or student)
- Your department
- Information about the Nominee
- Their name
- Their Department
- Your relationship to the nominee
- Your nomination: you may cut-and-paste your nomination from a Word document or type directly into the space provided.
- Letters of support: a minimum of two (2) letters of support are required, however we will accept up to three. You should solicit letters of support from faculty, staff, or students for this nominee.
After all of the questions have been answered, you will be able to view all of your answers. You also have the option to return to the form and edit your responses before you submit the nomination.
You may save an unfinished nomination before you submit it and return to it at a later time (to retrieve a saved nomination, return to the nomination form link.)
After you submit the completed nomination, you will be provided with a confirmation code. Please retain a printed copy for your records.
Please note:
- If a required field is not filled in, you will not be able to move on to the next page of the nomination.
- Any incomplete nominations (i.e. not submitted) will not be taken into consideration.
Important Note: Nomination materials will be shared with all nominees and included in personnel files, but may be edited from their original appearance and/or length.
