To ensure that VMMC provides accurate information about you and your professional affiliation, please fill in all fields except for the Session Description field. Please submit Presenter Data form by January 29, 2010.

The information provided VMMC is used by conference management to complete the final program, panel name cards, badges, and all correspondence. All participating presenters, panelists, facilitators, moderators and co-facilitators are asked to submit this form.
