To complete your nomination

1. Using the button below, download and save the form to your computer.
2. Fill out all applicable fields and save the completed form.
3. Send an email to Nancy Anderson, with the following documents attached:
• The completed nomination form
• Your personal narrative of the nominee's achievements that addresses the selection criteria and advocates for the nomination
• The nominee’s vitae, resume or biography (no more than 2 pages)
• Optional letters of support for the nomination addressing the award criteria and the nominee's merit are encouraged by the selection committee
