Diamond Award Nomination
The nomination period is now closed. Nominations re-open in September 2012 for the 2013 Diamond Awards. You may submit multiple nominations. If you have questions or would like a printed copy of the nomination form, please contact Nancy Anderson at (206) 685-2422.
To complete your nomination
- Using the button below, download and save the form to your computer.
- Fill out all applicable fields and save the completed form.
- Send an email to Nancy Anderson, with the following documents attached:
- The completed nomination form, which includes your personal narrative of the nominee's achievements.
- The nominee's vitae, resume or biography (no more than 2 pages)
- Optional letters of support for the nomination addressing the award criteria and the nominee's merit are encouraged by the selection committee.
